What is an HMB supplement?

HMB stands for beta-hydroxy-beta-methylbutyrate. The body makes it in two steps: leucine is first converted to a compound called alpha-ketoisocaproate (KIC), and a small fraction of KIC becomes HMB. The International Society of Sports Nutrition (ISSN) estimates that only about 5% of leucine is converted this way. Because eating enough protein to reach the amounts used in studies is not practical, HMB is sold as a supplement. Early animal tracer studies by Iowa State researchers confirmed that HMB in the blood comes from leucine.

A 2019 meta-analysis in the American Journal of Clinical Nutrition looked at 15 trials with 2,137 patients who had conditions causing muscle loss. HMB, often combined with other nutrients, produced a small gain in strength and a borderline gain in muscle mass, and no trial was rated at low risk of bias in every category.
