A GLP-1 pill is a tablet, swallowed once a day, that activates the same GLP-1 receptor as injectable medicines such as Ozempic and Zepbound. As of September 2026, the FDA has approved four oral GLP-1 products: Rybelsus and Ozempic tablets for type 2 diabetes, and the Wegovy pill and Foundayo for weight management. Three contain semaglutide and come with strict empty-stomach rules; Foundayo contains a different drug, orforglipron, and has no food or water restrictions. What is a GLP-1 pill?

GLP-1 is a gut hormone that raises insulin when blood sugar is high, slows stomach emptying and signals fullness. Most GLP-1 medicines are peptides, which the stomach normally breaks down, so for years they were only available as injections. There are now two ways to make an oral GLP-1 work. Novo Nordisk pairs semaglutide with an absorption enhancer, which lets a small share of the swallowed dose reach the bloodstream. Eli Lilly’s orforglipron is a small molecule rather than a peptide, so it is absorbed from an ordinary tablet. Oral GLP-1 for diabetes vs weight management

The most common mix-up is assuming any oral GLP-1 is a weight-loss pill. Each product is approved only for the uses on its own label:

  • Type 2 diabetes: Rybelsus and Ozempic tablets, alongside diet and exercise, to improve blood sugar control, and to lower the risk of heart attack, stroke and cardiovascular death in adults with type 2 diabetes at high risk. Neither is approved for weight management.
  • Weight management: the Wegovy pill and Foundayo, for adults with obesity, or with overweight and at least one weight-related condition, alongside a reduced-calorie diet and more physical activity. The Wegovy pill also carries a cardiovascular risk-reduction indication.

The trials behind the weight-management pills were separate, so their results cannot be ranked against each other. In OASIS 4, adults without diabetes on the Wegovy pill lost 13.6% of body weight on average over 64 weeks, versus 2.2% on placebo. In ATTAIN-1, adults on Foundayo lost up to 11.2% over 72 weeks, versus 2.1% on placebo. Both trials were funded by the drug makers. Lilly has also submitted orforglipron to the FDA for type 2 diabetes. There is no approved tirzepatide pill.

Why oral semaglutide has food rules and Foundayo does not

According to the Rybelsus and Ozempic tablets label, only about 0.4% to 2% of an oral semaglutide dose is absorbed, and food, other drinks and other pills taken at the same time reduce that further. That is why all three semaglutide tablets must be taken on an empty stomach with a limited amount of plain water, followed by a waiting period. Orforglipron is absorbed like most small-molecule drugs, so the Foundayo label has no food or water restrictions. The trade-off is interactions: orforglipron is broken down by the liver enzyme CYP3A4, so some common medicines change its levels, and the label advises a backup or non-oral birth control method for a period after starting and after each dose increase. Oral GLP-1 medicines still in development

Several oral GLP-1 drugs are investigational, which means they are not approved and are available only in clinical trials. Some examples as of September 2026:

  • Oral amycretin (Novo Nordisk): a single molecule that activates both the GLP-1 and amylin receptors. In June 2025 the company said it would move both the oral and injectable forms into phase 3 trials for weight management, starting in the first quarter of 2026.
  • Aleniglipron (Structure Therapeutics): a small-molecule oral GLP-1 that has reported phase 2 weight-loss results, with a phase 3 program planned.
  • Orforglipron for type 2 diabetes: already approved for weight management as Foundayo and under FDA review for diabetes.

Not every candidate makes it. Pfizer discontinued danuglipron, its oral GLP-1 for weight management, in April 2025. The company said liver enzyme rises across its safety database of more than 1,400 participants were in line with approved drugs in the class, but one participant had a potential drug-induced liver injury that resolved after stopping, and it ended development after reviewing the data and input from regulators.

Unapproved "GLP-1 pills" sold online and GLP-1 supplements

Search results for a GLP-1 pill also turn up products that are not FDA-approved medicines. They fall into two groups, and neither is interchangeable with the tablets in the table above:

  • Unapproved drug products. Sellers offer semaglutide or tirzepatide as drops, sublingual tablets, lozenges or capsules, sometimes labeled “for research use only”. In January 2025, South Carolina’s attorney general warned that such forms have never been evaluated by any regulator. The FDA says unapproved and compounded GLP-1 products are not reviewed for safety, effectiveness or quality, that counterfeits exist, and it sent warning letters to five online sellers in August 2026.   • Dietary supplements. Capsules and gummies marketed for “GLP-1 support” usually contain fiber, probiotics or berberine. The FDA’s advice is to get GLP-1 meds only with a prescription from a licensed prescriber, filled at a state-licensed pharmacy.

Side effects shared by oral GLP-1 meds

Because they act on the same receptor, the oral GLP-1 tablets share most of their safety information with the injections. The most common side effects are digestive, such as nausea, diarrhea, vomiting and constipation, mostly while the dose is being raised. All four labels carry a boxed warning about thyroid C-cell tumors seen in rodents and list warnings about pancreatitis, gallbladder problems, kidney injury from dehydration and low blood sugar when combined with insulin or a sulfonylurea.
