What the label says about timing in the body
The Ozempic label, revised in May 2026, says that 89% of an injected amount reaches the bloodstream, that levels peak 1 to 3 days after each injection and that steady levels are reached after 4 to 5 weeks of weekly use. With a half-life of about one week, semaglutide stays in the circulation for about 5 weeks after the last injection. The Wegovy label gives the same peak timing and says semaglutide remains for about 5 to 7 weeks after the last injection, which is why it advises stopping at least 2 months before a planned pregnancy.
Steady levels at the starting strength are not the same as full effect, though, because the strength is increased in steps.
Why the first weeks can feel slow
Every GLP-1 label in this article starts low and raises the strength gradually over weeks, to reduce nausea and other digestive side effects. The labels are explicit that the first step is a starting point. The Ozempic label lists its starting step separately from the strengths used for blood sugar control. The label for Rybelsus and Ozempic tablets says the starting strength “is not effective for glycemic control”, and the Zepbound label says its starting strength is for starting and is not approved for ongoing use. The Wegovy and Zepbound labels tell prescribers to consider response and tolerability when choosing the ongoing strength. We did not find a fixed week for judging response in either label.
When does Ozempic start working on blood sugar?
Semaglutide lowers blood sugar from the start of use, but the effect grows as the strength rises. The Ozempic label’s lab studies were done after 12 weeks, including the escalation period: compared with placebo, fasting glucose was 22% lower, glucose two hours after a meal 36% lower and average 24-hour glucose 22% lower.
HbA1c, the main diabetes measure, reflects average blood sugar over about the past 3 months, according to the National Institute of Diabetes and Digestive and Kidney Diseases. It changes slowly by design, which is why trials measure it at 26 to 40 weeks. In SUSTAIN 1, a Novo Nordisk trial of 388 adults whose type 2 diabetes was managed by diet and exercise alone, HbA1c fell by 1.45 and 1.55 points on two semaglutide strengths and by 0.02 points on placebo after 30 weeks, from an average of 8.05%. How long does Wegovy take to work for weight loss?
Two Novo Nordisk trials of Wegovy injection in adults without diabetes give the clearest picture:
- STEP 4 (JAMA 2021) gave semaglutide to 902 adults for a 20-week run-in: 16 weeks of escalation and 4 weeks at the maintenance strength. The 803 who reached that strength had lost 10.6% of body weight on average. This figure only includes people who stayed on and tolerated the drug.
- STEP 1 (NEJM 2021) followed 1,961 adults for 68 weeks. Average weight change was -14.9% with semaglutide and -2.4% with placebo. 86.4% lost at least 5%, 69.1% at least 10% and 50.5% at least 15%, which also means about 1 in 7 lost less than 5%.
In STEP 4, people who kept taking semaglutide after week 20 lost a further 7.9% by week 68, while those switched to placebo regained 6.9%. Loss therefore continued well past the escalation period, and stopping led to regain.
How long does Zepbound take to work?
The Zepbound label, revised in August 2026, says tirzepatide reaches peak levels a median of 24 hours after each weekly injection (range 8 to 72 hours), steady levels after 4 weeks, and has a half-life of about 5 to 6 days in people with overweight or obesity. It also notes that tirzepatide’s slowing of stomach emptying is largest after the first injection and fades over time.
- SURMOUNT-1 (NEJM 2022, supported by Eli Lilly) followed 2,539 adults without diabetes for 72 weeks, including a 20-week escalation period. Average weight change was -15.0%, -19.5% and -20.9% on three strengths vs -3.1% on placebo, and 85% to 91% lost at least 5%.
- SURMOUNT-4 (JAMA 2024, a Lilly trial) gave tirzepatide to 783 adults for a 36-week open-label lead-in; the 670 who completed it had lost 20.9% on average. Those who continued lost a further 5.5% by week 88, while those switched to placebo regained 14.0%.
